Thank you for taking the time to submit an entry application for the net.marketing excellence award. This award has been developed to recognize achievement, share and promote best practice in business use of the World Wide Web. Awards will be given out in both consumer direct and business-to-business categories. Winners will be announced at the 3rd annual DMA net.marketing conference & exhibition, February 28-March 2, 1999 in Los Angeles. Deadline for submitting entries is December 30, 1998.
Submissions will be judged by a panel of experts who will review submitted information and explore the components of the Web site online. Nominees may submit anything they feel will support their claim. It is at each company's discretion as to how much financial information they wish to submit. Financial information may be submitted as indexed figures.

Your application should include the following information:
- Award category
- Name of individual submitting the award application
- Title and organization of individual submitting the application
- Mailing address of applicant
- Phone number of applicant
- Fax number of applicant
- E-mail address of applicant
- Name of company or organization being nominated for award
- Web address of company or organization
- Background on the company Web site (ex. date site started, features, technology utilized)
- Goals and objective of the Web site (ex. revenue generating, cost savings, to establish a new brand, to extend current brand, to drive in-store traffic, to build awareness)
- Please describe what goals were met and how you went about meeting those goals
- If applicable, how did you involve the sales channel in meeting goals
- Describe any innovative aspects of the site
- Show any measurable or quantifiable results (i.e., increase in sales by 10% over budget, decrease in costs by 20% over past year)
