Part 3 -- Faculty Recommendation

Please make two copies of this form, have them signed, and attach each to a separate faculty recommendation.

TO THE STUDENT:

Your admission to the GW Madrid Study program will be based largely on academic information -- your transcript and this recommendation form. It is important, therefore, to have this form filled out by a faculty member in your major subject area.

We realize that you may have only recently decided on a major, may have taken only a few courses in the subject area, and may have been only in large classes where the teacher does not know you well. Nonetheless, it is better to discuss your program with one of these faculty members and have him/her fill out this form than to have the form filled out by someone who knows you better but teaches in a discipline you do not intend to pursue abroad.

Please sign the waiver, give this form to the appropriate faculty member, discuss the program if you have not already done so, and ask him/her to complete the form and send it to:
Office for Study Abroad and International Programs
The George Washington University
609 22nd Street, NW

I hereby waive my right to access the information on this form and ask that the form be filled out and sent to the GW Office for Study Abroad and International Programs.

TO THE FACULTY MEMBER:

The student named above is applying for the Madrid Study Abroad program through The George Washington University. We would appreciate your commenting on the student's academic ability as well as his/her social maturity and emotional strengths in terms of spending time studying overseas. We are particularly interested in your assessment of the student's academic motivation. It would be helpful if you could comment on the strengths of the individual as well as any weaknesses that could impede the student's ability to succeed overseas. We thank you in advance for your time and thoroughness in preparation of this recommendation. Please not that our receipt of this form (with written recommendation on reverse) is essential to the student's application for admi ssion. Thank you.

Academic area of internship request:

All students planning to participate in the internship program should fill out this internship request form. Please print out this form and attach it to a sheet of paper with your answers to the questions.

In addition, this proposal is to be agreed upon by the student and the home college Major Advisor and signed by both, as well as by the student's Academic Dean or other home college official responsible for approving the internship for transfer credit.

The information provided below will assist the director in finding a suitable internship placement. While many students undertake field work in their major area, this is not always required as long as the student's academic background and experience are s uitable to the internship requested.

A written project is required for completion of the internship experience. It is meant to integrate your academic experience abroad with your work activities. Your final grade for the internship includes a grade for the written project and a grade for a n oral defense of your research.

Please answer the following questions on a separate sheet of paper.

1. Describe in general the kind of field work you would like to undertake. Is there any type of specialized work or experience that you would like to have included as part of your internship? Do you have any specific topics in mind? What kind of daily res ponsibilities would you like to have?

2. Summarize your academic background and any experience you have that is relevant to your internship request.

3. What are your education and career objectives and what education or career-directed benefits do you hope to gain from this internship?

4. If possible, please provide a general idea for the written project you would like to undertake.

5. Has your home college imposed any particular conditions or requirements (e.g. length, format) for your written project? If yes, please explain.

TO BE COMPLETED BY THE HOME UNIVERSITY MAJOR ADVISOR:
I hereby approve the nature of the requested internship.

It is consistent with the academic policy of this institution to consider the transfer of the internship credit toward the satisfaction of this student's degree requirements.
